Who We Are

JaxCareConnect is a joint collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents find a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to healthcare in order to support best possible health outcomes.

Navigating healthcare can be an overwhelming process. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a healthcare provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their healthcare journey.

JaxCareConnect aims to better inform uninsured neighbors about their healthcare options so that all Duval County residents have access to comprehensive primary care—regardless of insurance status or ability to pay.

JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.

Our process is simple! Individuals, or anyone on their behalf, seeking more information on our services may complete a patient questionnaire and someone from our team will reach out within 48 hours.
