How Connection to Care Helped Ms. Williams Build Stability

When Ms. Williams* first connected with JaxCareConnect, she was navigating a demanding season of life. A Jacksonville native, she was balancing part-time work while continuing her education. With a background in social work and plans to become a paralegal, she was focused on building a better future. But several barriers made that path more difficult.

At the time, Ms. Williams was searching for mental health support, affordable housing, and access to consistent medical care. While she had received some services elsewhere, she knew she needed more comprehensive, coordinated support to truly move forward.

She first heard about JaxCareConnect at a Feeding Northeast Florida food pantry event. That moment became a turning point.

Through JaxCareConnect, Ms. Williams was connected to critical resources that addressed both her immediate and long-term needs. A referral to the Mental Health Resource Center led to therapy and medication management services that she describes as empowering. She was also connected to a safe and stable housing opportunity through Ability Housing, helping her move closer to her goal of independent living.

Beyond her own care, Ms. Williams received support that extended to her family. She was connected to resources to ensure her aging grandmother could remain safe at home with appropriate medical support. She also received assistance that helped her maintain employment while managing her health and responsibilities.

Throughout the process, Ms. Williams credits her Patient Health Advocate for making a lasting impact.

“Kelsey made me feel supported and empowered,” she shared.

Today, Ms. Williams says she has achieved her goals of improved mental health and affordable, independent living. With a stronger foundation in place, she is continuing her education and moving forward with confidence.

Reflecting on her experience, she offers simple but powerful advice to others who may be struggling:

“Call JaxCareConnect now.”
