Happy New Year! We hope 2023 was as amazing for you as it was for us. We grew, we achieved, we connected – and we could not have done any of it without your support.
The ways in which JaxCareConnect grew in 2023 varied. We had 1,089 patients referred to our program. We got 919 patients established in primary care with a Duval Safety Net Collaborative free clinic partner. And we made 795 referrals, on behalf of our patients, to community partners whose services relate to the Social Determinants of Health. We also grew by way of team members! This past year, we welcomed Breana Barr. She came on in June as a part-time Patient Health Advocate – providing our team with extended coverage like evenings and weekends.
In 2023, we celebrated quite a few achievements! The first being the end of our pilot. At its core, a pilot program involves carefully planning, executing, and evaluating a program to gather necessary data and insights before making significant investments in or commitments to it. Now that the pilot is successfully completed, JaxCareConnect will continue its work to increase access to healthcare here in Duval County. We also over-achieved (pun intended) by way of fundraising. Our original fundraising goal for our Giving Tuesday campaign was $500. We passed our goal within 2 hours of the campaign starting. So, at the end of the campaign, we ended up raising $936 – nearly double what we originally predicted. Over achieving is just in our nature, it seems.
Lastly, but certainly not least, we connected with various community organizations in 2023. The biggest connection being with the City of Jacksonville. Funding from the City of Jacksonville was approved on September 26th, 2023, in the City Council Strategic Plan meeting to go to JaxCareConnect and the rest of Duval Safety Net Collaborative.
This past year was nothing short of a doozy. We’re so excited for what 2024 holds. Thank you for supporting our work all this time.
