Social Determinants of Health Screening:
What are Social Determinants of Health (SDoH)? Social determinants are non-medical needs that have a big impact on an individual’s long-term health and quality of life outcomes. They are the conditions that surround the places where we live, learn, work and play. Examples of social determinants of health include access to healthy food and clean water, affordable and safe housing, quality childcare and education, transportation, and the community and culture around us. The JaxCareConnect team offers SDoH screenings for our current and potential clients to identify current obstacles to care so that we may overcome them together. Current and potential clients are invited to complete the assessment using the survey below and someone from our team will connect with you.
