
In 2021, Mr. Schmidt was referred to the JaxCareConnect program while experiencing dangerously high blood pressure and on the verge of a heart attack. The team at Ascension St. Vincent’s recognized the urgency of his situation and connected him with us to help him find a primary care medical home that could support his long-term health.
For six years, Mr. Schmidt had gone without access to medical care. After the company he worked for was acquired and underwent mass layoffs, he lost his job – and with it, his health insurance. That loss triggered a cascade of challenges: he eventually lost his home and his truck, making everyday life and accessing care even more difficult.
Through JaxCareConnect, Mr. Schmidt was paired with a dedicated Patient Health Advocate, who helped him establish primary care at the Sulzbacher Clinic. Due to years without treatment, Mr. Schmidt required specialized care across multiple fields – including Nephrology, Urology, Hematology, and Pulmonology. Fortunately, he was able to receive this comprehensive care at no cost through WeCareJax.
Beyond medical needs, Mr. Schmidt also needed help accessing basic resources. His Patient Health Advocate connected him with community services, including transportation assistance, clothing, and durable medical equipment.
